Overview

Mindyra believes that in order to achieve optimal care of a person affected by mental illness or a learning disability, the holistic needs of the individual and his/her family must be met. In order to achieve this optimal care, Mindyra will collect some information about you and your family to help our case managers facilitate your needs.

Mindyra is committed to protecting your privacy and security of the information you provide us.  We collect user information in order to improve your experience with us, support your ability to access and edit your payment information, and to communicate with you about the products and services which you have purchased as well as our other products and services.  

This privacy policy (“Policy”) describes our practices regarding information gathering and dissemination.  This Policy explains what information we collect, and how it may be shared with third parties to provide services on your behalf.  

Information that May be Collected

A) Information You Provide To Us

Mindyra collects information that you voluntarily submit.  In order for you to use Mindyra, we may ask you to register and provide us with personal information.  We may also collect credit card information to bill you for any products or services you purchase from us.   This personal information may include your name, e-mail address, user ID, domain name, URL, credit card number, mailing address, and telephone number.  We may also collect demographic information such as your age, gender, zip code/postal code, preferences, and interests.

Additionally, whenever you enter information on Mindyra’s website for the range of services that Mindyra offers, that information is stored and accessible to Mindyra for future use.  For example, we may use your account and e-mail address to communicate with you about our services.  

If you share information available through Mindyra with another individual, you acknowledge and accept responsibility for your decision to provide the individual with access to potentially sensitive information.

B) Website Usage Information

When you use Mindyra and its services, we may track information on how you interact and use our website through server log information, cookies and other similar tools of Internet tracking.

Server Log Information (IP Addresses):  An IP address is a number automatically assigned to your computer whenever you access the Internet.  All computer identification on the Internet is conducted with IP addresses, which allow computers and servers to recognize and communicate with each other.  Mindyra collects IP addresses in order to conduct system administration, report Aggregate Data (as defined below) to affiliates or partners, and to conduct web site analysis.  Mindyra will also use IP addresses to identify any users who refuse to comply with the Terms and Conditions of Use agreement, and to identify users who threaten Mindyra’s services, website, users, clients or others.

Cookies and Web Analytics:  Mindyra places a text file called a ‘cookie’ in the browser files of your computer. Cookies are pieces of information that a website transfers to an individual's hard disk for record-keeping purposes.  Mindyra uses cookies during your online session to determine whether a user is logged in or not, and to improve performance of the website. Mindyra does not use cookies to store passwords or sensitive information. These cookies do not contain any personal information. You may disable cookies in your browser but doing so will restrict your access to only public pages and you will no longer be able to access many features on Mindyra.  In addition to cookies, some web analytics tools used by Mindyra place a single-pixel GIF file on a computer as a tracking indicator.

How We Use Your Information

Mindyra may use your information for various internal purposes.  Mindyra will store data for as long as it is necessary to provide services to you and others, as well as for legal protections or as otherwise required by applicable laws and regulations.

Mindyra does not rent, sell or share personal information about you with other people or nonaffiliated companies without your permission, except for the following circumstances:

  1. Providing your information to affiliates, subsidiaries, and trusted partners who work on behalf of or with us under confidentiality agreements.  Although these companies may use your personal information to assist us in our operations, they have no independent right to share your information and must abide by confidentiality and security obligations.
  2. Mindyra may provide de-identified information about you and others, such as usage information and trends, to third parties.  This information, commonly referred to as Aggregate Data, is pooled information of many individuals, and is stripped of any data that could potentially identify you as an individual.   This information is commonly used for research purposes.
  3. Mindyra may provide information about you in order to properly respond to subpoenas, court orders or legal process, or to establish or exercise our legal rights or defend against legal claims.
  4. Mindyra may disclose elements of your personally identifiable information to another business entity in the event of a plan to merge with or be acquired by such business entity, or to any organization involved with the merger, transfer, or sale of our assets or business. 

How to Update, Disable, or Delete Your Information

If you wish to update your personal information on file with Mindyra, please do so by updating your contact information under your account settings.

In the event you no longer wish to use Mindyra and its services, your account can be disabled from the Mindyra website.  Disabling your account does not notify Mindyra that you no longer wish to receive communications from us.  If you no longer wish to receive communication from Mindyra, please unsubscribe and opt-out of receiving these communications by using the link in an e-mail communication.

Security

Mindyra recognizes the importance of its customers’ information and strives to protect the security of your information.  In so doing, Mindyra takes reasonable security measures to protect your information. We do not guarantee that Mindyra will be safe or secure, so please use Mindyra at your own risk.  

Children’s Personal Information

Mindyra is not suitable for use by children.  We do not knowingly sell or market to, solicit information from, or sell products or services for purchase to people under 18.  If you are a parent of a child under the age of 18 and your child has used our websites, you may contact us by using the contact information below and we will delete any personal information about your child as soon as reasonably possible.  

HIPAA

Mindyra is not a health care provider, health plan or health care clearinghouse as such terms are defined under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations, all as amended (collectively, “HIPAA”).  As such, Mindyra is not subject to HIPAA.  However, as is set forth above, we recognize the importance of your information and strive to protect its security.

Notification of Changes to Privacy Policy

Our Policy may change from time to time.  We will always post an up to date version on our website, with the latest revision date posted at the top of the page. We encourage you to periodically review this policy to see if any changes affect you.
